Data provider class

Experts,

I have 5 offline adobe forms. A portion of the form fields have been planned to be built dynamically since these fields vary across the 5 forms. Based on form type, I will fetch fields from a custom table & display them dynamically on form layout(For EHSM module).

Should i create 5 different custom data provider classes for these 5 forms & write my dynamic field population logic inside them?
