smartforms drawing boxes

Hello

I got hard time to find a way to draw simple   empty  boxes in smartforms??  thing that  it is easy to do in sapscript   or Adobe.

I just want   to draw   something   identical to this and I will just pt variables  in .  with chadow on the header  and all sides of the rectangles are solid
