Dropdown Values refreshing after selection to the first item in the list

Hello Guru's,

I have 2 Dropdowns, one for Action Type and other for Action Reason.

While creating PCR form, I select a Action Type and a Action Reason Defaults to a value.

Example: Action Type: Position Attribute Change Action Reason is Defaulted to: REORGANIZATION. Action Reason have 4 values, Reorganization, Cost Center Allocation Change, Demotion, Temporary Assignment. If I select any other Action Reason, say Demotion and click on Next the value is defaulting to Reorganization.

Script for Action reason is attached.

Please help in fixing the issue.
